Adoption Contract

I/We (hereinafter referred to as Adopting Party) attest to the facts stated in the submitted Adoption Application and consent to adopt , a(n) from Companion Parrots Re-homed (hereinafter referred to as CPR).

The following terms and conditions constitute a legal contract for the adoption of the parrot referred to above (hereinafter referred to as “parrot”) between CPR and Adopting Party. These terms and conditions are entered into and agreed upon by both parties and they acknowledge the legal worth of this contract.
Please initial if in agreement to:
1. Contribute an adoption fee in the amount of $ . I/We understand that the adoption fee may be refunded up to two weeks after this date.

2. Not breed the parrot

3. Keep the parrot in a 100% smoke free environment

4. Not take the parrot outdoors unrestrained (must use cage or harness)

5. Not allow parrot to interact with other pets unless a bird of similar size

6. Provide an annual veterinary examination by a CPR approved avian veterinarian

7. Feed a well-rounded diet of high quality pellet, fresh vegetables and fruit, grains and nuts

8. Allow follow up home visits as needed

9. To keep CPR informed of current email address, physical address and phone number.

10. To notify CPR immediately if parrot is lost or stolen and make every effort to locate the parrot.

11. Contact CPR immediately if for any reason the parrot cannot remain in my/our custody to discuss arrangements. The parrot will not be sold, adopted, or given to another party.

I/We understand that if for any reason it is no longer a suitable match we will return the parrot without prejudice. Doing so does not necessarily disqualify us from adopting again.
ENFORCEMENT OF CONTRACT
It is understood that failure to meet the contractual obligations stated above voids this contract and the parrot will be immediately returned to CPR. Any problems noted during follow up home visits or conversations with CPR representatives must be addressed and resolved or the parrot will be immediately returned to CPR.
RELEASE OF LIABILITY AND INDEMNIFICATION
Adopting Party promises and agrees to be solely responsible for this animal and to indemnify and hold harmless CPR from any and all claims of liability for the conduct of this Parrot on or after the date of this adoption and agreement of contract. I understand and agree that CPR makes no guarantees about the parrot’s temperament and is not responsible for future damages or injuries caused by the parrot.
